ANNEX 1 MANDATORY CONDITIONS

No supply of alcohol may be made under the Premises Licence at a time when there is no Designated Premises Supervisor (DPS) in respect of the Premises Licence or at a time when the DPS does not hold a Personal Licence or their Licence is suspended.

Every supply of alcohol under the Premises Licence must be made or authorised by a person who holds a Personal Licence.

Each individual in the premises to carry out a security activity must be licensed by the Security Industry Authority unless the premises or part of the premises where they are present are being used are wholly or mainly a restaurant or guest house, theatrical performances, plays or a Gaming Licence is for the time being in force in respect of those premises
ANNEX 2 CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE

The premises will operate in accordance with plan submitted to the Licensing Authority on 4/9/2012

A CCTV system shall be installed, maintained and operated at the premises in liaison with and to the satisfaction of Lancashire Constabulary and shall be used to record during all hours that the premises are open to the public

There shall be provided at the premises door supervisors who are registered with the Security Industry Authority to such a number as the management of the premises consider are sufficient to control the entry of persons to the premises and for the keeping of order in the premises when they are used for a licensable activity.

Fire extinguishers and an adequate first aid kit shall be maintained and be available at all times the premises are open to the public

A system of emergency lighting shall be maintained to the satisfaction of Lancashire Fire and Rescue Service

An automatic fire detection system shall be maintained to the satisfaction of Lancashire Fire and Rescue Service

An external 90 litre waste bin shall be available outside the premises which shall be emtied regularly, and there shall be periodic litter patrols, details of which shall be recorded in a log book maintained for that purpose

Notices shall be clearly displayed requesting patrons to dispose of rubbish responsibly

Only necessary packaging shall be provided

Section 34(5e) Gaming Act 1968 or Category C Gambling Act 2005 machines shall be sited within sight of the bar
