ANNEX 1 MANDATORY CONDITIONS
No supply of alcohol may be made under the Premises Licence at a time when there is no Designated Premises Supervisor (DPS) in respect of the Premises Licence or at a time when the DPS does not hold a Personal Licence or their Licence is suspended.
Every supply of alcohol under the Premises Licence must be made or authorised by a person who holds a Personal Licence.
ANNEX 2 - CONDITIONS IMPOSED IN ACCORDANCE WITH THE OPERATING SCHEDULE & UNDER PARAGRAPH 6(6) & 6(8) SCHEDULE 8 LICENSING ACT 2003
The challenge 21 policy will be operated by all staff in relation to all sales of age restricted products.
ANNEX 3 CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
A CCTV system which covers both the inside and outside of the premises will be installed, operated and maintained to the satisfaction of the Lancashire Constabulary.
That during the currency of the licence the premises will be monitored during opening hours by a CCTV system installed and maintained on the premises to the satisfaction of Lancashire Constabulary
That the images recorded by the CCTV system shall be stored in a safe place and retained in an unedited form for a period of not less than 31 days. Recordings shall be made available to an authorised officer of the licensing authority or the Police.
Litter bins of an appropriate size shall be provided outside the premises whilst open for licensable activities.
